Open Enrollment Is Now Closed

Welcome to your 2025 open enrollment.  Open enrollment is a crucial period, providing you with the opportunity to make important decisions regarding your health benefits. The enrollment window will be open from November 25 to December 8, 2024.

Please be aware of the following options and instructions:

Maintain Your Current Plan: If you wish to continue with your current medical benefits plan, check the box to carry over your current from 2024 to 2025.

Make Changes to Your Plan: If you would like to make changes to your current medical benefits elections, review the Blue Cross Blue Shield, Blue Care Network, Delta Dental, and Vision plans and make your selection.

Waive Coverage: If you decide not to take advantage of the ACHC benefits, it is imperative that you select the ‘Waive Coverage’ option.

This year, employees who only require dental and/or vision coverage can enroll in Delta Dental and/or VSP (vision). For those needing Blue Cross Blue Shield or Blue Care Network medical plans, dental and vision coverage is automatically included.

Blue Cross Blue Shield and Blue Care Network Medical, Dental & Vision

2025 BCN PCP Focus Platinum W/Expanded Abortion

The Blue Care Network of Michigan PCP Focus Platinum plan is a type of health insurance coverage that emphasizes primary care provider (PCP) coordination. In this plan, members are required to choose a primary care provider who will be their main point of contact for healthcare services. This provider coordinates all necessary referrals and manages the overall care of the patient.

The “Platinum” designation typically indicates a higher level of coverage with lower out-of-pocket costs compared to other tiers like Bronze, Silver, or Gold. This can mean lower deductibles, copays, and a higher percentage of costs covered by the insurance after the deductible is met.

ACHC will contribute $200.00 each month towards the total cost of your plan. For instance, if your plan costs $500.00 per month, you would deduct $200.00, making your monthly expense $300.00. This amount would be divided into bi-monthly payroll deductions of $150.00 per payroll.

Delta Dental &Vision

2025 Delta Vision

The Delta Vision plan, offered by Delta Dental, provides comprehensive vision care coverage to its members. This plan is designed to help individuals and families reduce the cost of routine eye care and prescription eyewear. Members of the Delta Vision plan have access to a broad network of optometrists and ophthalmologists, ensuring that finding a provider is both easy and convenient.

Key features of the Delta Vision plan include coverage for annual eye exams, which often come with a low co-pay, making regular check-ups affordable. The plan typically covers a significant portion of the costs for prescription glasses and contact lenses, with specific allowances for frames and lenses provided on an annual basis.

Additionally, the Delta Vision plan may offer discounts on elective vision correction surgery, such as LASIK, as well as special rates on additional pairs of eyeglasses or enhancements like progressive lenses and anti-reflective coating. Members can also benefit from out-of-network coverage options, although utilizing in-network providers maximizes the benefits.

Delta Vision emphasizes not only the importance of vision care as part of overall health but also strives to make managing vision care benefits straightforward through user-friendly online tools for checking coverage details, finding providers, and submitting claims. This plan is an excellent choice for those who value both the health of their eyes and the clarity of their vision.

The Delta Dental of Michigan Group PPO Plan is designed to offer comprehensive dental coverage to groups and organizations, providing a broad range of dental services at affordable rates. As a Preferred Provider Organization (PPO) plan, it offers flexibility in choosing dentists and dental specialists either within or outside Delta Dental’s extensive network, with greater savings occurring when in-network providers are used.

Key features of the plan include preventive care such as cleanings, exams, and x-rays.

ACHC does not offer a contribution towards this plan.
